The places with the lowest death tolls also happen to be places that don't experience a lot of international travel, or are primarily rural so the spread of disease isn't as widespread. It's not a very lethal disease, given the relatively low fatality rate and all, but it's highly infectious and lasting. The only people who are really worried about it are those who live cheek by jowl with others. 1% of your average city's population is a hell of a lot of people, and since cities control the media, quarantine policy is based around their needs.
It's hardly a conspiracy, just a bunch of burgers and limes panicked over the possibility of urban collapse. They've spent decades touting the advantages of urban living, and now it's backfiring. It was a similar deal when swine flu was going around, the difference is that healthcare systems were more robust back then and people trusted these institutions to do their jobs.
