Same have happened in Norway in a case. The people "redacting" only added black background to text, probably assuming a printed version would be redacted so the PDF would be redacted too. Or something like that, not sure what they thought. But could be they assumed only printed copies would be released.
Usually redacted documents are printed then scanned making them hard to copy text from (bi-product of scanning printed document) and needs to be run through OCR to be searchable.

is the blacked-out text recorded in a separate document elsewhere in a higher-security facility?
Reason to print out and scan is so you avoid these mistakes 100% (it is basically feilsafe way to react). Too many office workers otten thinks Black text on Black background = redacted; save file.
The power of lazy or ignorant or incompetent government workers strikes yet again.
So many ways to stop this from happening. Yet, nothing was done.
Possible chance other documents were processed by the same worker with that failure. Only for a time.
